Health First Colorado will send a renewal packet either in the mail or to your email telling you to check your PEAK mailbox several weeks before your renewal due date. If you use the Health First Colorado app, and have opted in to push notifications, you will get a notification letting you know when it’s time to take action. You can also check the Health First Colorado mobile app or contact Health First Colorado Enrollment to find out your regional organization at 1-888-367-6557 (State Relay: 711).

You should always bring your photo ID with you when you see your health care provider or go to the pharmacy. You do not need a copy of your Health First Colorado card to get medical services. You can also view your Health First Colorado card from the Health First Colorado mobile app. On Aug. 30, 2020, the new Health First Colorado mobile app will be available in the Apple App Store and Google Play Store. Members can download the app directly from the app stores, or by updating their PEAKHealth app. Effective Aug. 1, 2021, Health First Colorado members who reside outside of the following counties, Adams, Arapahoe, Boulder, Broomfield, Denver, Douglas, Jefferson, Larimer, and Weld, will need to contact their local transportation provider directly to schedule non-emergent transportation services, including subscriptions. If you are concerned that you may have missed a letter with this information, you can find out who your regional organization is by checking the Health First Colorado mobile app or by calling Health First Colorado Enrollment Monday to Friday, from 8 a.m. to 5 Members can submit the renewal form signature page through the following modalities: online through CO.gov/PEAK, on the Health First Colorado mobile app, on paper by mailing, faxing, or by bringing the signed renewal form to their local county department of human/social services.
